Referring an Elder to the Visiting Volunteer Program
Thank you for the care and concern you demonstrate by referring an isolated elder to us. In order that they may receive and benefit from our volunteer services please review the information below.
In order to avoid unnecessary use of both yours and staff time, before you refer them to our Visiting Volunteer Program we ask you to be certain that the elder concerned fits the following criteria:
ELDERS ARE ELIGIBLE FOR OUR PROGRAM IF THEY ARE:
- Aged 65 or older
- Live in their own home (not a Board and Care or Skilled Nursing Home)
- Receive less than two social visits per month (e.g. have no family/close friends that visit.)
If, after review of the referral form, the elder appears eligible for our program, a LBFE staff person will visit them in their home in order to acclimate them to the program and assess their home situation. At that point, at the staff members recommendation, they will be placed on our “to be matched” list, and we will look for a volunteer to match with them. The process is a very personal one, dependent on the individual elder and the available volunteers, and there is no way to measure when a match will take place.
In the meantime, after the staff assessment, they are eligible for our monthly home-visiting events and are invited to participate in our other activities.
