Know an isolated elder who could benefit from one or more of our programs? If so, please complete the form below and we will begin our evaluation process. Elders must be 65+ or 60+ living with a disability, receive less than two social home visits from friends or family per month, and live in San Francisco. Exceptions are made on a case-by-case basis. LBFE expressed a desire to offer respite care for family members caring for older adults in their homes. However, LBFE's services are tailored to older adults who live alone and do not live with family members. Referrals for older adults interested in our medical escort ONLY cannot be processed.
